Unless you are staying at a relatively high-class hotel, do not expect anyone to accept credit cards. Most people I came across wanted pesos, not U.S. dollars. In the medium-large towns you can find a bank with a separate area (normally available 24hours daily) housing an ATM where you can take pesos straight from your bank account – but there’s a catch… a 5-7 dollar fee, ouch!
Regarding car rentals, the daily rates were good price, but the insurance is a killer, I had to pay $19 per day in addition to the rental. I would recomment getting a written confirmation of the insurance rate when you book, so there are no questions when you return the vehicle. We got a Nissan Tsuru (small 4-door sedan) from Advantage that was suitable for all the rough terrain we went on.
On a side note, I would not recommend going to San Blas during September because of the unbearable heat.
